WYANDOTTE INDIANS REGISTRATION INFORMATION – 2010 SEASON

New Player registration scheduled for the 2010 season:
You will need to attend the session to complete the necessary paperwork and to pay the necessary fees for your child(ren) to be assured a place on the appropriate squad.
Saturday May 22, 2010.   This open registration session will take place at the Copeland Center, 2306 4th Street, Wyandotte. See designated times listed below.

                    
            
       ***PARENTS, PLEASE BRING YOUR FOOTBALL PLAYER FOR PRACTICE JERSEY SIZES!!!

Registration fees for the 2010 season are as follows:
$85 for one child; $50 for each additional child; $45 for Varsity players/cheerleaders, with a family registration cap of $225.  (Varsity players/cheerleaders will be considered the first child in families of two or more).
There will also be a $75 equipment/ three event volunteer deposit per family. If your equipment is not returned at the end of the season, clean and in good condition and, if the family has not volunteered at three events throughout the season, the family will not receive a refund of their $75 deposit.

PLEASE NOTE:  THERE WILL BE A $30 CHARGE FOR ALL RETURNED CHECKS!!

A PARENT OR LEGAL GUARDIAN MUST REGISTER FOR A CHILD. We must have the parents/guardians signature notarized on the registration form. A notary public will be available during the registration sessions. PARENTS/GUADIANS, BE SURE TO HAVE YOUR DRIVER’S LICENSE WITH YOU SO THAT THE NOTARY PUBLIC CAN VERIFY YOUR SIGNATURE. ALSO, PLEASE BRING YOUR HEALTH INSURANCE CARD.

IF YOU ARE A LEGAL GUADIAN, WE WILL NEED TO SEE THE GUADIANSHIP PAPERS AND WE WILL NEED A PHOTOCOPY OF THE PAPERS FOR OUR RECORDS.

IF YOU ARE REGISTERING A CHILD FOR THE FIRST TIME; PLEASE BRING THE ORIGINAL AND ONE COPY OF THEIR BIRTH CERTIFICATE.

ALL PLAYERS AND CHEERLEADERS MUST HAVE A PHYSICAL BEFORE THEY ARE ALLOWED TO PRACTICE.  Parents are responsible for obtaining their child’s physical, more information will be provided at registration, as well as physical forms.  Please note that you must have your family doctor sign the physical form; no signature stamp will be accepted.  Any registration questions, please contact Julie Butterfield @ 734-286-5394.
